Admission

Joan Bracco has been our Marketing and Admissions Coordinator for almost 4 years. Not new to Marketing or Admissions, Joan has worked for other nursing facilities locally along with many years working with the retired and elderly within our county. We are very privileged to have her on our team here at Ridgeview.

One of her primary functions is to prepare the paperwork on each resident prior to their admission to Ridgeview Elder Care Rehabilitation Center. The following is the admission process that must be followed for each referral to the facility.

Pre-Admission:

  1. From the Hospital - please sign the release to authorize the hospital to release medical records to our facility.

  2. Facility Tour - an appointment is not always necessary to receive a tour. However, if you want to see the Marketing/Admissions Director, Joan Bracco, or the Business Office Manager, Nanette Haller please call 814-236-0600 and schedule an appointment. We are available and willing to visit with you in your home, hospital, or where ever is convient for you to assist you with information regarding services available and admission information. What ever your needs may be we are here to assist you!

At the Time of Admission:

    The following information will be required:

    Completed admission application; medical insurance identification card(s): Medicare, Veterans Administration, Health America, Amera Health, Geisinger Health Plan, Geisinger Gold,Blue Cross, Blue Shield, AARP, Medicaid, PACE, prescription plan, access card, supplemental insurance and ambulance membership card, etc.

  1. If possible, please provide the following:

    • Advanced Directive, such as a living will.

    • Power of Attorney documents.

    • Legal Guardianship documents.

    • Responsible family member or power of attorney will need to sign admission forms if the resident is unable to do so.

    • Information regarding Church and Pastor.

    • Information regarding all next of kin, (names, addresses, and telephone numbers).

    • Information regarding a Funeral Director and prepared burial papers.

    • Proof of age:

      One of the following proofs of age: PACE card, current or expired drivers license, PA ID, birth certificate, baptismal certificate, passport, or military discharge papers. If you are unable to obtain one of these proofs of age, please call your Social Security Office to request a "Proof of Age" letter. Clearfield County Social Security Office phone number is: 814-371-8099. Please bring this letter to the receptionist as soon as you receive it. We will be happy to answer any questions you may have concerning admission to our facility. We look forward to meeting and working with you and your family.

Social Service

Our Social Services Department is a primary link between the nursing center and the family. They get to know each resident and to help him or her to adjust to the facility. Our Social Services personnel are always available for private counseling or to address any concerns that may arise. You may contact Marisa Rothrock at 814-236-0600 and she will be happy to assist you.
